In this course, we promise to guide students in the activities performed in the safest way possible.

Participants and parents who enroll their children in sports programs have to recognize, however, that there is an inherent risk of injury. Insist that participants follow safety rules and instructions, which are designed to protect the safety of the participants.

Please read this form carefully and be aware that registering your minor child in this class, you will be waiving and releasing all claims for injuries that your child may suffer during this activity.

WAIVER AND RELEASE OF ALL CLAIMS
